Terms and Conditions for Dyslexia Diagnostic Assessment

The dyslexia diagnostic assessment is a comprehensive evaluation conducted by a qualified specialist teacher to identify dyslexia or related learning difficulties.

The assessment includes:

- Pre-assessment consultation (via phone or online).

- Full diagnostic assessment, which includes cognitive and literacy testing.

- A detailed written report with recommendations.

Fees and Payment

- The total fee for the dyslexia diagnostic assessment covers the assessment, report, and any consultation related to the results.

-A 50% deposit is required to secure the booking. The remaining balance must be paid no later than 48 hours before the scheduled assessment. Without advanced payment, the assessment will not take place.

- Payment can be made via bank transfer. Bank details will be provided upon booking.

Cancellation and Rescheduling Policy

-Parents can cancel or reschedule their assessments without penalty if they provide at least 48 hours' notice. However, deposits cannot be refunded.

- In the event the specialist teacher needs to reschedule, the client will be offered an alternative date at no additional cost. If the specialist teacher needs to cancel and can no longer carry out the assessment, a full refund of the deposit will be given.

Assessment Results and Reporting

The assessment report will be completed and provided to the client within 3 weeks after the assessment. It will be written to the required standard and adhere to the guidelines outlined by the SpLD Assessment Standards Committee (SASC). 

- The report will include an analysis of the results, a clear diagnostic decision regarding dyslexia, and recommendations for further support or accommodations.

- The report is intended for the client’s personal use and may be shared with schools or other professionals at the client’s discretion. However, the specialist teacher is not responsible for how third parties interpret the report.

Confidentiality

- All personal data and assessment results will be handled in accordance with the Data Protection Act (2018) and the General Data Protection Regulation (GDPR).

- The information shared during the assessment process will remain confidential and will not be shared with any third parties without the client’s explicit consent, except as required by law.

-Following the assessment and compilation of the final assessment report, all client questionnaires, test papers and personal data will be permanently deleted/destroyed. 

-The assessor will hold the final assessment report for 6 years after the assessee’s 18th birthday. During this time, you will be able to ask for an electronic copy of the report.  However, we would strongly recommend that you keep a copy of your report securely.

Client Responsibilities

- The client is responsible for providing accurate background information during the pre-assessment consultation and completing any pre-assessment forms.

-The client is responsible for ensuring their child has an up-to-date eye test from the last 6 months before the assessment day. Without this, the assessment cannot take place.

-The client must ensure that glasses or any coloured overlays that the child uses as their ‘ normal way of working’ are brought to the appointment.

-the client must agree to the payment terms above.

-the client must provide contact details, including phone number and email address.

-The parent/guardian should ensure that they provide accurate and up-to-date information regarding any known safeguarding issues that might affect the assessment process.

-The parent must stay on-site for the duration of the assessment

-Where co-occurring needs mean that extra accommodations may be required, this should be discussed with the assessor prior to assessment.

Assessor Responsibilities

-The assessor must prioritise the client's well-being during the assessment. This includes being sensitive to any emotional or psychological impact the assessment might have on the client, especially younger individuals.

-The assessor must take appropriate measures to provide a supportive environment and recommend further support when needed. The assessor must also ensure that all testing conditions (whether in-person or online) are appropriate and conducive to reliable results. The environment will be free from distraction.

-The assessment environment, whether in-person or online, will be made as safe and secure as possible. For in-person assessments, this includes ensuring that the venue is safe, private, and free from any risks.

-If the assessment is conducted online, the assessor will take steps to ensure that the platform used is secure and that client privacy is protected.

-The assessor reserves the right to pause or postpone an assessment if it becomes clear that the individual requires a break or is finding the process difficult.

-The report must be shared with the client within the agreed time frame.

-The assessor must explain the assessment process clearly to the client, including what is expected during the assessment and how the results will be used.

-All assessments must be conducted in accordance with ethical guidelines established by the British Dyslexia Association.

-The assessor must act impartially, ensuring the assessment process is fair and free from bias.

-The assessor is committed to ensuring the safety and well-being of all clients, particularly children and young people. The assessor holds an up-to-date, enhanced Disclosure and Barring Service (DBS) check. Evidence of this can be provided upon request.

-The assessor holds an Assessment Practising Certificate (APC) with the British Dyslexia Association. The Assessor will ensure that they receive SASC approved CPD and keep their professional qualification up to date and renewed when necessary.

-If safeguarding concerns arise, the assessor reserves the right to terminate the assessment and report the matter to the relevant authorities.

Liability

- The specialist teacher will take all reasonable care to ensure the accuracy and quality of the assessment and report.

- The assessment is based on the child’s performance during the testing process. The specialist teacher cannot guarantee a specific outcome or diagnosis.

- The specialist teacher is not liable for any consequences resulting from the client’s use of the assessment report. The specialist teacher/ assessor may recommend onward referral to other professionals as a result of the diagnostic assessment but is not liable for any further diagnosis that may arise from third parties.

Complaints Procedure

- If the client is dissatisfied with any aspect of the service, they should notify the specialist teacher in writing within 14 days of receiving the report.

- The specialist teacher will aim to resolve any complaints within 30 days.

Terms and Conditions

One- to one Specialist Dyslexia Lessons 

Payment 

In order to make the most out of the specialist tuition, there is a minimum commitment of one 1-hour session on a weekly basis. 

All lessons are term time only and payable in advance at the beginning of each half term.  

For in-person clients, the first session may be paid in cash on the day, or a one-off bank transfer can be made for online clients; a contract will be established thereafter if the parents/ students wish to proceed with tuition. 

Once a contract is in place, an invoice for tuition for that half term will be emailed to the parents and should be paid before the next session. Invoices will then continue to be emailed at the end of each half term to pay for the following half term’s tuition in advance and should be paid within 7 days. The account details for payments are provided on the invoice.  

Tuition fees and term dates will be in line with my own term dates, which may differ from county to county. Please see my term dates. Tuition dates will also be included on the invoices. 

If invoices are regularly not paid on time, I reserve the right to offer your child’s session time to another student on my waiting list and terminate the agreement without notice. 

Lateness

Appointment times should be adhered to where possible.  In the event of lateness, please notify me as soon as possible. If possible, I will move the session to a slightly later start time. However, please understand that this cannot be guaranteed as I teach/ assess other students, and as a result, I may only be able to offer the original scheduled time for the lesson. Thus, only the remainder of the session will therefore take place. There will be no refunds for missing part of a lesson due to the student’s lateness. 

In the event that I am late for a session, I will offer a) a partial refund, b) to reschedule if the delay is significant c) for the session to run over the usual scheduled time, if agreed by both parties. I also reserve the right to cancel the lesson, in which case I will refund or reschedule based on your preference. 

In cases where I am running late, I will always attempt to call you or the school at the earliest convenience. 

 Learning Space 

For appointments at my property or at the student’s home, parents must stay on the premises and within sight of the student at all times. At Peak Dyslexia Practice, parents will wait in the waiting area for the duration of the session. Lessons at my property will take place in a dedicated ‘classroom’ area connected to the waiting area. 

For online lessons, parents must be in the same room as the student or close by and within sight of the student. 

When appointments take place at the child’s school, I will discuss the most appropriate space for the lessons with the school. 

The environment, whether in-person or online, should be made as safe and secure as possible. This includes ensuring that the venue is safe, private, and free from any risks.

If the lessons take place online, I will take steps to ensure that the platform used is secure and that client privacy is protected.

Ring Doorbell 

Please note that I use a Ring doorbell camera at my property to provide a safe and secure environment. Access to the Ring doorbell images/ videos is only permitted by myself and my husband (homeowners).

Cancellation Policy  

If you must cancel a session and notify me 24 hours before, I will try to reschedule at a mutually convenient time. I will not offer refunds for missed lessons and may not be able to reschedule without notice. If I cancel the session for any reason, then I will fully refund the cost of the missed lesson. 

Contract Notice:  

The first session can be paid in cash on the day for in-person clients and a one-off bank transfer may be made for online clients; if parents/students are happy to proceed, a contract will be put in place. The notice period to terminate the contract will be 4 weeks for both parties. I also reserve the right to terminate the contract immediately based on non-compliance with agreed terms, disruptive behaviour, or any other serious breach of the agreement. In this case, fees for unused sessions will also not be refunded. 

Teacher- Parent Consultation 

Time to discuss the student’s progress is built into each session, with a 50-minute lesson and 10 minutes consultation time. Any new developments which arise at school or at home which would be beneficial for the tutor to know can be emailed in advance of the lesson and discussed at the session, either before or after the tuition. Equally, if you have any questions about the tuition/ resources/ strategies, etc., please do not hesitate to email or ask on the day of the lesson. If the discussion requires more time, I would happily continue the conversation on a scheduled telephone call, time permitting. 

Resources 

The student must ensure they arrive at the lesson with all the necessary materials. Parents must provide appropriate folders and wallets to keep any worksheets/ reading and spelling cards safe and ensure the student brings these to every lesson. I will provide a list of any extra resources that may be necessary. I may email resources to online clients or provide a list of useful resources to keep at home for the sessions.

Specialist Teacher Responsibilities

-Assess learning in the first session and develop an appropriate learning plan for each student.

-Deliver specialist dyslexia tuition using a multi-sensory, cumulative literacy programme at a mutually agreed day and time.

-Discuss the student’s progress at the parent's request at a mutually agreed time.  

-I am committed to ensuring the safety and well-being of all clients, particularly children and young people. I hold an up-to-date, enhanced Disclosure and Barring Service (DBS) check. Evidence of this can be provided upon request.

-I must maintain appropriate insurance cover and an up-to-date DBS certificate. Regularly engage in safeguarding training.

-Maintain and update skills and knowledge in dyslexia teaching and related areas through CPD activities. CPD may include attending workshops, seminars, or courses and keeping up to date with current research and changes in practices.

-Provide notice to terminate the contract in writing.  

-Provide at least a term’s notice should there be an increase in tuition fees.

Parent/ Client Responsibilities

-Pay fees as set out in the contract.  

-Arrive to lessons on time.

-Ensure all resources are looked after carefully and are brought to the lesson.

-Give 4 weeks’ notice, in writing, of termination of the contract. The student should continue to attend lessons during the notice period.  The parent agrees to pay the full fee during the notice period.

-Stay on-site at all times 

-Provide accurate contact details.
